Liability waiver & release *
I am the parent/legal guardian of the student named above. I understand that participation in SAVI Prep, Inc. programs, training, events, and activities ("Activities") involves inherent risks — including strenuous physical exertion, physical contact, and the risk of minor or serious injury. In consideration of my daughter being permitted to participate, I knowingly and voluntarily: (1) assume all risks associated with participation, whether known or unknown; (2) release, waive, and agree to hold harmless SAVI Prep, Inc., its directors, employees, coaches, volunteers, and affiliates (the "Released Parties") from any and all claims, liabilities, or demands arising out of or related to my daughter's participation, including those caused by the ordinary negligence of the Released Parties, to the fullest extent permitted by law; (3) certify that my daughter is physically fit to participate, or that I have disclosed any relevant condition above; and (4) authorize SAVI Prep, Inc. staff to seek or provide emergency medical care for my daughter if needed, and agree to be responsible for any resulting costs. This agreement is governed by the laws of the State of Arizona. I have read this release in its entirety and agree to its terms freely.
